Job Opening: Acute Care Station (ACS) Navigator
Location: San Antonio, TX
Shift: 11:00 PM – 7:00 AM, Monday to Friday to start (eventually shifting to weekends)
Training Hours: Monday-Friday, 8:00 AM – 4:00 PM for two weeks
Compensation: $17/hour
About the Role:
The ACS Navigator plays a key supportive role, assisting Life Safety Officers in working with clients who require medical or social service attention, ensuring they connect with the San Antonio Fire Department (SAFD) and other essential services. This is not a medical role—Navigators do not administer medicine or medical treatment. Instead, they provide guidance and assistance, facilitating access to services like medical appointments, psychiatric care, and community resources.
Key Responsibilities:
- Locate ACS clients seen by SAFD and assist them with follow-up actions, such as navigating medical and psychiatric appointments.
- Sign in, stay with, and support ACS clients during on-site and off-site medical appointments.
- Provide bus passes and snacks to clients attending appointments, promoting continuity of care.
- Ensure thorough documentation of all client interactions, meeting Haven for Hope and grant requirements.
Qualifications:
- Experience in a supportive, community-oriented role; home health or first responder experience preferred but not required.
- Strong computer skills including proficiency in Microsoft Word, Excel, and PowerPoint.
- Ability to learn and adapt to custom information systems and concurrent documentation methods.
- Organizational skills with an ability to manage tasks and contribute effectively to a team.
- CPR and Trauma-Informed Care training provided; must meet physical requirements to complete training.
Physical Demands:
This role requires physical capability to assist clients and complete training, including lifting up to 20 pounds and supporting up to 55 pounds. Activities include standing, bending, stooping, and other moderate physical activity.
Interview Process:
Qualified candidates will interview on-site with supervisor, followed by a one-hour shadowing session to ensure understanding of the role.
