Job Description - Published: Care Coordinator
This role involves providing comprehensive support services to individuals residing in permanent supportive housing, focusing on client-centered care and service coordination. Duties encompass a wide range of activities including outreach, intake, assessment, service planning, and ongoing monitoring.

Responsibilities:
Conduct outreach and establish rapport with clients to assess program suitability. Obtain necessary consents and authorizations for client participation.
Assist with intake procedures, ensuring accurate completion of necessary documents.
Facilitate access to temporary housing and provide support during transition to permanent housing.
Coordinate services to meet immediate needs such as food and transportation.
Conduct regular assessments and update service plans to track progress and modify goals.
Connect clients with health care, mental health, and substance use disorder services as needed.
Support clients in managing medication regimens and attending appointments.
Assist with income generation, employment, and educational opportunities.
Provide education on crisis intervention, tenant rights, and budgeting.
Collaborate with property managers and partners to resolve housing stability issues.
Coordinate transition planning for clients with changing housing needs.
Participate in meetings and case conferences with partners and supervisors.
Foster collaborative partnerships with community organizations to enhance service access.

Qualifications and Requirements:
Bachelor’s Degree in Social Work, Psychology, or related field.
Minimum of one year of experience in social services or case management.
Knowledge of case management systems and best practices.
Familiarity with mental health, chronic health issues, and substance use disorders.
Strong organizational skills and attention to detail.
Ability to work independently and manage a caseload effectively.
Proficiency in Microsoft Word and Excel.
Valid driver's license and access to transportation.
Commitment to confidentiality and ethical care practices.
