We are seeking a compassionate and dedicated Case Manager to join our team. The Case Manager will be responsible for providing comprehensive case management services to our clients, ensuring they receive the support and resources needed to achieve their goals.
Key Responsibilities:
Conduct assessments to determine client needs and develop individualized service plans.
Provide ongoing support and advocacy for clients, including coordinating services and resources.
Maintain accurate and up-to-date case files and documentation.
Collaborate with other team members and community partners to ensure holistic support for clients.
Monitor and evaluate client progress, adjusting service plans as necessary.
Participate in team meetings and training sessions to stay informed about best practices and resources.
Qualifications:
Bachelor's degree in Social Work, Psychology, or a related field (Master's degree preferred).
Minimum of 2-5 years of experience in case management or a related role.
Strong understanding of community resources and social services.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a team.
