Member Care Coordinator – Remote

Are you passionate about helping people navigate their healthcare options? We are seeking a Member Care Coordinator to join our dedicated team. In this role, you will provide guidance to members regarding available benefits, services, and programs, while assisting with care coordination to ensure their healthcare needs are met efficiently and effectively.

Key Responsibilities:
Conduct outreach calls to educate members on available healthcare benefits, services, and programs.
Complete health needs assessments and refer members to relevant health management programs.
Identify gaps in care and proactively assist members in accessing necessary medical services, including provider appointments.
Manage system work queues to assess member eligibility, review prior case activity, and coordinate outreach efforts.
Assist with research and resolution of inquiries related to healthcare processes, incentives, and program participation.
Support administrative tasks such as processing Health Risk Assessments and managing incoming faxes.
Work independently in a remote-first environment while collaborating with a fully developed support team.
Qualifications & Skills:
Education: Associate’s degree in education, communication, healthcare, or a related field OR equivalent work experience.
Experience: Minimum of 2 years in a customer service or healthcare support role.
Ability to work independently with minimal supervision while contributing to a team environment.
Strong problem-solving, decision-making, and organizational skills.
Effective time management with the ability to handle multiple priorities.
Excellent oral and written communication skills.
Familiarity with medical terminology and experience in a healthcare setting preferred.
Digital literacy assessment required.
